How to Make an Access Request
 

Applications for Information from Government Institutions

Under section 5 of The Freedom of Information and Protection of Privacy Act, every person has a right to apply for access to records that are in the possession or under the control of a government institution.  Section 6 of the Act states that application for information is to be made in the prescribed form to the "government institution" in which the record containing the record is kept.

Applications for Information from Local Authorities

Under section 5 of The Local Authority Freedom of Information and Protection of Privacy Act, every person has a right to apply for access to records that are in the possession or under the control of a "local authority".  Section 6 of the Act states that application for information is to be made in the prescribed form to the "local Authority" in which the record containing the record is kept.

Request for Review

If you are not satisfied with the response of the government institution or local authority or health trustee you can apply to the Commissioner for a review.
